Payment is due at the time of your scan. $490 covers everything — the scan, the radiologist's interpretation, and the report to your provider.

Who self-pay is right for

Self-pay works well if you're uninsured, your plan doesn't cover imaging at Clarus, or you'd simply rather know your total cost upfront. With a high deductible, $490 self-pay is often less than your in-network cost — and you can pay with your HSA or FSA. Call and we can talk through your specific situation.

Out-of-network claims

Clarus Imaging accepts self-pay patients and can help you pursue out-of-network reimbursement if your plan covers out-of-network services. Out-of-network providers generally can't submit claims on your behalf — you'll submit the claim yourself, using the information below.

Before your scan

Check your out-of-network benefits with your insurer, including:

  • Your out-of-network benefits, if any
  • Your out-of-network deductible, and whether you've met it
  • The allowed amount for the service — ask your referring provider for the CPT code(s) to give your insurer
  • Your out-of-network co-insurance (usually a percentage of the allowed amount)

After your scan

  1. Ask us for a superbill — a document detailing the service, with the diagnosis and procedure codes your insurer needs to reimburse you.
  2. Submit an out-of-network claim to your insurer, following their specific submission instructions.

Insurance plans that cover out-of-network services typically reimburse 50–80% of the cost, once you've met your deductible. Your insurer's Member Services line can confirm your plan's specifics.
